This customized dental savings plan is our alternative to dental insurance benefits. It has been designed specifically to provide greater access to quality dental care at an affordable price. Not only will this non-insurance plan save you money, it will finally provide you with the unlimited coverage that you deserve.
There are:
- NO yearly maximums
- NO deductibles
- NO claim forms
- NO pre-authorization requirements
- NO pre-existing condition restrictions
- NO outside consultants dictating your treatment
- NO waiting periods
- NO denied coverage
Program Exclusions and Limitations
- The plan Effective Date is the date when the Enrollment Fee is paid.
- Plans are active for 365 days from the Enrollment Date.
- Plans are not transferable to another party or family member.
- Patients are responsible for making and keeping appointments.
- For perio patients who require moren involved hygiene therapies, the cost of a routine cleaning will be applied towards your therapy with a copay.
- Exams, x-rays, cleanings and other features of the plan must occur within the plan year and cannot be carried over to the next year.
- Discounts under this program shall not apply to any treatment started prior to enrollment or after membership expires.
- This Health and Wellness Plan is not dental insurance and is only accepted at our office. Therefore, Health and
- Wellness Plan benefits cannot be transferred to another office or specialist.
- Plans are in lieu of insurance plans and cannot be used with other dental coverage.
- Plans do not apply to dental treatment needed as a result of injury when disability, workman's compensation or a lawsuit are involved.
